Community Data Roundtable (CDR) provides its Users with the option to enroll in a self-service password reset program through the use of SMS text messages sent to individuals’ cellular phones.

If you choose to use this service, you must provide your cell phone number. Text messages may be sent to your cellular device to initially verify the ownership of a cellular phone and perform the subsequent password reset services through SMS text messages. As such, message and data rates may apply from your mobile carrier, and you agree to pay any such charges by enrolling in the SMS program. CDR does not charge for the use of its password reset program.

You may opt-out of this service at any time. To cancel this service, you must log in to your CDR DataPool™ account and remove the phone number you registered for program enrollment. You can re-enroll at any time.

Carriers are not liable for delayed or undelivered messages. Password reset codes sent via SMS may not be delivered to you if your phone is not within range of a transmission site or if sufficient network capacity is unavailable at a particular time. By using this service, you acknowledge that SMS password reset codes may not be received timely and that your wireless carrier does not guarantee that alerts will be delivered.

If a text message is not available or not received in time to perform a successful password reset, you may choose to use one of the following alternative methods for password reset:

If you receive unsolicited codes for self-service password reset or links pretending to be self-service password reset links, please contact the CDR Support team immediately at support@communitydataroundtable.org .

CDR reserves the right to terminate the password reset program at any time without notice.

CDR is committed to the privacy of your information and will apply industry-standard controls and exercise care to safeguard your provided information. To view CDR privacy policies, please visit Privacy Principles.

By enrolling in this service, you agree to the above mentioned terms and conditions and represent that you are the owner of the wireless device for which you have authorized enrollment in the CDR password reset program and are responsible for messages sent to and from the device.
